Individual, Couples, and Family Therapy: The fee for services is 100.00 for the first session and 80.00 per session for subsequent sessions. Sessions typically run 45-50 minutes except for the first session, which may run 60-75 minutes.
Group Therapy: The fee for services is 40.00 per session. Sessions will run 50-60 minutes.
The client is responsible for payment at the time of each session. I accept cash/check only.
Exceptions to the above fees are if you are utilizing your EAP benefit and have up to ____ sessions at no charge, or, you are utilizing your health insurance benefit. If you have co-pay you are responsible for payment at the time of each session.
If you have health insurance please let me know. Managed care plans require that I bill your insurance directly. Most insurance companies cover the services of licensed clinical social worker, but there may be other restrictions such as waiting period, deductibles, co-payment requirements and/or pre-authorization. If you insurance does not pay a claim, you agree to responsible for payment for service rendered.
Insurance panels that I am contracted with currently are Blue Cross Blue Shield of Oregon, Optum (PBH and UHC), MHN, Integrated Behavioral Health, Cascade Health, Pacific Source/Reliant Behavioral Health and Cigna.
